Canthal tilt is the angle of the line connecting the inner corner of your eye (medial canthus) to the outer corner (lateral canthus). If the outer corner sits higher than the inner, that's a positive tilt — looks alert, 'sharp.' Level is neutral. Outer corner lower than inner is a negative tilt, which reads tired or sad. That's the entire metric: one slope across one eye.
